Seeking a Level AHP2 Social Worker for the provision of assessment and treatment services for inpatient, home and community rehabilitation patients.

This opportunity is to join the Northern Adelaide Local Health Network on a short-term contractual basis, working fulltime Monday to Friday.

On behalf of our client in South Australia, we are seeking a Level AHP2 Social Worker to deliver high quality, assessment and treatment services for inpatient, home, and community rehabilitation patients across Northern Adelaide. Applying clinical experience, knowledge, and professional competence to plan, implement and evaluate comprehensive and integrated services to the needs of the clients.

Key Responsibilities

  • Using a competent level of professional knowledge and skill to provide assessment, interpret finds plan and implement intervention in collaboration with patients carers and other health professionals
  • Seeking guidance and supervision from experienced staff when more complex problem solving, professional decision making and practice skills are required
  • Ensuring documentation is consistent with service policy and practice
  • Undertaking comprehensive discharge planning in conjunction with other team members
  • Maintaining and developing clinical and professional knowledge and skills
  • Working collaboratively with team members of other disciplines
  • Participating actively in Multi-Disciplinary service teams and projects
  • Participation in departmental meetings
  • Participation in research and evaluation of the Social Work service
  • Undertaking data input, and participating in reviewing and evaluating data produced by clinical information management system (CME)
  • Providing support to undergraduate students and to work experience students
  • Participating in, and facilitating the delivery of, collaborative team work and effective continuous learning and professional development

About You

  • Degree or equivalent qualifications with eligibility for full membership of the AASW
  • Proven competency and knowledge in Social Work assessment and treatment techniques
  • Demonstrated ability to communicate effectively both verbally and written
  • Excellent time management skills to effectively manage a caseload
  • Ability to work effectively in inter-disciplinary teams
  • Demonstrated competency in the clinical management and treatment of a broad range of conditions
  • Understanding of Work Health and Safety principles and procedures
  • Understanding of the Australian National Safety & Quality Health Service Standards
  • Strong knowledge of Social Work professional standards

Excellent casual rats up to $58-67 per hour plus super (based on experience)
